GENERAL REMARKS

The personal data owners being defined as relevant person under the 6998 No Law on Protection of Personal Data (“LPPD”) (hereinafter to be referred to as “Application Owner”) have been granted under the Article 11 of LPPD to make certain requests regarding personal data processing.

The applications to be made to our Company as data responsible under the Article 13/1 of LPPD in relation with such rights should be made to us in writing or through other methods stipulated by the Personal Data Protection Board (“Board”).

It is critical the personal data we are saving about you to be accurate and updated. Therefore, please inform us when there is a change in your personal data. You may, under the LPPD in relation with yourself:

–           learn whether personal data are processed,

–           request details regarding the data we have collected,

–           the purpose why such personal data are collected and how the same are used, and the

third parties within the country and abroad to whom personal data are transferred or

–           request correction of personal data if they are processed in a deficient or defective

manner, updating of personal data or deletion of the same if reasons, which require them to be processed, in a manner to comprise notices to be given to third parties to whom personal data have been transferred.

In this framework, the applications to be made to our Company “in writing” may be submitted as follows by printing this form and writing “Information Request Regarding the Protection of Personal Data” on the envelope or as subject:

• through personal application of the applicant,
• through public notary or reply paid registered letter,
• sending an e-mail by using registered electronic mail to our company.

A response to your application shall be given as soon as possible within 30 days at the latest under relevant provision of LPPD. We would like you to know that we may contact with you as Data Responsible to conclude your application even if the information and documents you have submitted are deficient or incomprehensible.

This application form is drafted for the purpose of determining your relation with our corporation and responding to your application accurately and in time by determining your personal data being processed by our corporation and their nature, if any. Our Company keeps its right to claim additional information and documents to determine identity / power for the purpose of eliminating legal risks, which may arise from sharing in violation of principles, procedures and merits provided under the Law and particularly ensuring the safety of your personal data. In case the information you share in the form are not accurate and updated or an unauthorised application has been made, our Corporation does not accept responsibility due to claims arisen from inaccurate information or unauthorised application. All liability arisen from illegal, misleading or inaccurate applications shall be borne on you.
